Why a Care Manager ?

Now more than ever before, our aging population is living longer and with more ailments that quickly diminish their ability to safely care for themselves. You, as a loved one to an aging family member, naturally find yourself worried about them but find yourself unsure of how you can help. This is where a Care Manager comes in. A Care Manager is a certified health care professional who can take on every aspect of your loved one’s care so that you can have peace of mind, and your loved one can have safety, dignity and purpose in life.
Care Management starts with a conversation. We start by listening to your concerns and fears for your aging parent or spouse. We then conduct a full assessment of your aging loved one. The assessment includes meeting them in their living setting, making skilled observations and standardized measurements of their functional abilities, and listening to them. In the world of Care Management, your loved ones are the client, and you are the customer. As Care managers, we do everything we can to preserve the wishes of our clients; we do that by safeguarding their well-being with a comprehensive plan.
What is Our “Plan”?
We look at the following aspects of our client’s life:
Their wishes–they need to continue to feel a sense of purpose.
Their strengths and limitations--they need to continue to engage in their strengths and have support for their limitations
Their budget–protecting assets and assembling care that fits their budget
Their advance directives–assigning trusted people to carry out their wishes should they themselves not be capable of expressing them. This is essential, and everyone needs to be informed.
Finding the right experts–from a physician, to an attorney, to the person who drives them to appointments–everyone needs to understand how to engage with our clients. We have a network of experts who we call upon and recommend tailored to our to clients’ individual needs. We are by their side as needed to facilitate interactions, and we train the caregiver team to carry out the plan optimally.

We view our role as building a village around our clients. We involve every member of our client’s circle of loved ones, because that is the village. We acknowledge that chronic stress, while it is loving stress, can harm our customers and we meet you where you are at–we seek to understand how you want to be involved, and based on that understanding, we give you the resources you need to cope and facilitate your involvement.

Junk mail and phone calls ask them to “donate.” TV ads convince them to buy unnecessary products and services. We set up protective shields to make sure your loved ones are safeguarded against these predatory tactics.
