By engaging North Shore Care Management for care management services, you agree to the following terms: • Services are provided for the purpose of care coordination, advocacy, and support as described in your individualized care plan. • Fees are due as agreed upon in your service contract or invoice. • Cancellations require [24/48] hours’ notice unless otherwise stated. • We are not a medical provider and do not diagnose, prescribe, or provide hands-on medical treatment. • We reserve the right to modify services or fees with reasonable notice.
